Here's the setup. When food arrives in your gut, your gut releases hormones that travel up and tell your brain to ease off. One of them is called GLP-1. That's the signal semaglutide - Ozempic - copies, and it's the reason that drug became a household name.

Retatrutide doesn't copy just that one. It works on three signals together: GLP-1, plus two more the papers call GIP and glucagon. Tirzepatide (Mounjaro) added the second one. Retatrutide adds the third. The whole question researchers are testing is whether pulling three levers at once does more than pulling one or two.

It comes from Eli Lilly, the papers call it LY3437943, and it has reached Phase 3 - the big final round of human trials that comes before any regulator decides whether to approve something. It hasn't been approved anywhere yet.

Three signals, three different jobs. Here's what each one actually does - the science in the published papers, not a description of what this vial does.

GLP-1

The 'that's enough' signal. Your gut releases it when food shows up, and it does two things researchers care about: it tells your brain you're full, and it slows how fast your stomach empties. Ozempic works on this one, and only this one.

GIP

The other gut signal, firing after the same meal. Its job leans towards insulin - how the body handles the sugar and fat that just arrived. Mounjaro added this one on top of GLP-1, and that is what made it different from Ozempic.

Glucagon

The odd one out, and the reason Retatrutide exists at all. The first two signals are about intake. Glucagon is about spend: when the body needs energy and none is arriving from food, glucagon is the message that goes to the liver saying release some. It is the mirror image of insulin - insulin puts energy away, glucagon takes it back out. That third lever is the whole bet.
